All clients must sign the New Client Forms and Client Discloser. Please read the below Client Disclosure thoroughly and proceed to fill out the New Client Form (also provided on this page). If you prefer to do this the old-fashioned way, you can print both documents, complete them by hand, and bring them to your first scheduled appointment.
